New Student Enrollment/Kindergarten Registration / Fall Check In

Magical Murray's Fall Check-In Process

ONE DAY ONLY ~ WEDNESDAY, AUGUST 15, 2012

3:00 PM to 7:00 P.M.

All Parents/Guardians must attend to verify Fall enrollment.

Please bring the following REQUIRED forms/items:

DUSD Student Health/Emergency Information Form: Current telephone numbers and a minimum of two emergency contacts for your student must be included.

DUSD Residency Verification Form: TWO proofs of residency must be included. (one from each category)
- PG&E Statement or DSRSD Statement OR homeowner’s/renters insurance statement
- Mortgage Statement/papers or escrow papers or Current rental/lease agreement
- All items must show the name and address of the parent/guardian

DUSD Network Policy/Photo Release

The forms are fillable PDFs, so you can type your information and print it out, and then bring with you to Fall Check In. You may also print the forms and hand write the information (please print legibly). There will also be some printed forms available in the Murray Office in August, when you pick up your Fall Check In Packet.

Also, please review the following items that ALL families in Dublin schools must read prior to "Fall Check-In" in August. The "DUSD Acknowledgement" Form will be verificatioin that you have read the documents below:

Donate to science fund/Friends of the Library/classroom supplies/safety funds

August 13 & 14 between 8:00 AM and 4:00 PM
