Prospective Students & Parents


In order to register your student, please bring photocopies of the documents listed below and these two forms completed (Click here for Registration Form)  (Click here for Household Enrollment Form)
  • Immunization records
  • Transcript (Final Report Card for incoming 9th graders)
  • STAR Testing Results
  • Special Education Plan (IEP) if enrolled in Special Education classes
  • California High School Exit Exam results for a student in the 11th or 12th grade.
    If registering during current school year:
  • Withdrawal form from previous school with grades
Proof of Residency
  • Final escrow papers showing purchase of home and parent or legal guardian's name. OR
  • Current rental agreement with a cancelled check or receipt for deposit showing the manager's name and phone number or legal guardian's name; AND
  • Current PG&E bill or deposit for PG&E service turn-on showing current address and parent or legal guardian's name; OR
  • Current DSRSD bill showing correct address and parent or legal guardian's name
Health
  • Documentation regarding special medical concerns your student may have.
Guardianship
Legal documentation that verifies full time guardianship if student does not live with a parent. Guardian must be present to register the student.
 
Verification of Birth Date of Student (one of the following):
  • Birth certificate
  • Passport
  • Baptismal Certificate
