    This form is needed on file for each parent that would like to be able to drive students in addition to your own student on a classroom field trip. Just click on the form, print it out, complete it and attach a copy of your car insurance declaration page showing all the required information. Then submit it to your student's classroom teacher for processing. 
    This form is required beginning January 17, 2017 for all volunteers attending a school fieldtrip. 
    This form needs to be completed anytime a family moves.
    Every student should have this form on file with the school office and with an original parent signature in the case of an emergency.  This form has to be updated each year at Fall Check In. 
    Parents can submit this form when updating information.
    If your student is going be absent for more than 5 days and less than 20 days, parents may apply for an Independent Study for their student(s). The request needs to be submitted to the OFFICE at least 2 weeks before the trip. The request must be approved by both school Administration as well as the classroom teacher. Once approved, the work for the trip will be processed and parents will be notified to pick up work the day before you leave. 
    If your student is leaving Amador Elementary, please complete this form and submit to the Amador Registrar, Mrs. Lotus Lee at Leelotus@dublinusd.org prior to your leaving.
