If your student is leaving Amador Elementary, please complete the Withdrawal Form (Digital Form). Once you complete this online form please also email our enrollment secretary/registrar Tabitha Leni - firstname.lastname@example.org with your student details.
For ANY Change in Information (Residency change, Emergency contact change, phone/email changes etc.) please email our Registrar Tabitha Leni at email@example.com with the updated information.
This form is required to be on file for each parent that volunteers at school, drives on field trips or helps with school activities. Please click on the link, complete the form, sign, attach a photocopy of your drivers license and submit to your student's classroom teacher.
This form is needed on file for each parent that would like to be able to drive students in addition to your own student on a classroom field trip. Just click on the form, print it out, complete it and attach a copy of your car insurance declaration page showing all the required information. Then submit it to your student's classroom teacher for processing.
Independent Study Request Forms - Revised Dec 2018
If your student is going be absent for 5 days or more, and less than 10 days, parents may apply for an Independent Study for their student(s). The request needs to be submitted to the OFFICE at least 2 weeks before the trip. The request must be approved by both school Administration as well as the classroom teacher. Once approved, the work for the trip will be processed and parents will be notified to pick up work the day before you leave.

In order to ensure that parents have appropriate access to their children's classrooms while avoiding unnecessary disruption to classroom environments and student instruction, the following standards of practice are recommended.
